Please contact the school nurse if your child has any life-threatening allergies. If your child requires an epinephrine auto-injector such as an EpiPen or Auvi-Q, please make sure all the paperwork is filled out and the medication is brought to the Health office by the first day of school. You can find the medication forms on the medication tab. In addition, please fill out the FARE action plan and the allergy questionaire that pertains to your child and bring them in with the medication forms. You will find these forms to the right of this page.
